Breath-holding spells are involuntary responses triggered by intense emotions like frustration, pain, fear, or anger. Understanding why toddlers hold their breath helps explain that these episodes are not done on purpose, and children cannot control them. Episodes typically last 10–30 seconds and almost always resolve without intervention.

 

There are two main types:
1. Cyanotic (Blue) Spells
Triggered by anger or frustration. Child cries intensely, exhales, and stops breathing before inhaling again.
2. Pallid (Pale) Spells
Triggered by sudden fear, shock, or pain. Child may briefly become pale or limp.

Both look scary, but are usually harmless.

Why Breath-Holding Spells Happen

Common causes include:

  • overtiredness
  • overstimulation
  • minor injuries
  • feeling overwhelmed
  • frustration during transitions

Most children outgrow breath-holding by age 4–6.

What to Do During a Spell

Stay Calm

Lay child on their side. Remove nearby hazards. Don’t shake or splash water.

Wait It Out
Children resume breathing naturally.

Comfort Afterwards
Once breathing returns, hold and reassure them. They are frightened, not manipulative.

What Not to Do
Don’t yell or discipline during or immediately after.
Don’t assume it is attention-seeking.
Don’t blow on the child’s face (it can worsen panic).
